About the Role UBMD Primary Care is seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to support our Orchard Park outpatient clinic. This role is ideal for someone who enjoys direct patient care, thrives in a fast-paced clinical environment, and brings strong communication and organizational skills to every interaction. Experienced LPNs and new graduates are encouraged to apply. What You’ll Do In this role, you’ll use your clinical, technical, and patient-care skills to support providers and ensure efficient clinic flow. Responsibilities include: Clinical Care & Patient Support Escorting patients to exam rooms and completing clinical intake Obtaining vitals and documenting patient history in the EMR Administering immunizations, injections, and point-of-care tests Assisting providers with exams, procedures, and treatments Preparing and maintaining exam rooms, equipment, and supplies Packaging and processing specimens for laboratory analysis Care Coordination & Communication Communicating care plans, test results, and instructions to patients Providing patient education based on diagnoses and provider guidance Managing e-scribe refill requests and patient portal messages Ensuring charts and records are complete and accurate for each visit Operational Support Reviewing and completing provider orders Scheduling tests or follow-up appointments when needed Maintaining vaccine records and clinic supply inventory Supporting phone triage and call coverage as needed Following HIPAA, OSHA, infection-control, and safety standards Skills That Lead to Success - We’re looking for candidates who can demonstrate: Core Skills Strong clinical judgment and patient-care abilities Excellent communication and customer-service skills Ability to multitask and stay organized in a busy clinic Flexibility and adaptability to changing patient needs Teamwork, professionalism, and reliability Technical Skills EMR proficiency Vital signs and clinical intake Immunizations and injections Point-of-care testing Specimen handling Basic office technology (phones, scanning, faxing)
